According to the Maine Medical Marijuana Act, registered qualifying medical marijuana patients are allowed to obtain replacement medical marijuana cards from the Maine Office of Cannabis Policy. Patients typically require replacement medical marijuana cards when their current cards are stolen, lost, or destroyed.
Per Section 2425 (6)(F) of the Maine Medical Marijuana Act, in the event of medical cannabis card loss, a medical cannabis cardholder must notify the Office of Cannabis Policy within 10 days of losing the card.
To obtain a replacement medical cannabis card in Maine, complete a Registry Identification Card Change or Reissue Form. Submit the completed application and applicable fee to:

It costs a non-refundable fee of $10 to obtain a replacement medical cannabis card in Maine. The Office of Cannabis Policy does not accept personal checks and cash as payment for this fee. The $10 fee must be paid using a cashier's check or money order, made payable to the Treasurer, State of Maine, submitted in person or sent to the OCP's mailing address:

According to Section 2425 of the Maine Medical Marijuana Act, the Office of Cannabis Policy will issue an applicant a new registration identification card with a new random identification number within 5 days of receiving a completed replacement card request form and the required application fee.
